Amino Acids Are Critical for Good Health

Amino acids are the building blocks of protein and are critical for good health. They provide our energy and maintain healthy skin and hair, build muscles, and boost our immune system among others. The body needs twenty amino acids, of which; nine are called essential amino acids and are not manufactured in our bodies but provided through diet. These are histidine, isoleucine, leucine, lysine, methionine, phenylalanine, threonine, tryptophan, and valine.

Foods that contain all of the essential amino acids are known as complete proteins and are found in a variety of foods including red meat, chicken, fish, eggs, milk, yogurt, cheese, tofu, quinoa, chia seeds and buckwheat. Experts explain animal proteins are the most easily absorbed and used by the body.

Foods that contain some of the essential amino acids are referred to as incomplete proteins and include things like nuts, seeds and beans, some grains, and fruits and vegetables. If you do not eat animal foods, you should eat a large variety of plant foods that contain some of the amino acids to ensure you are getting your daily requirement. Too little protein can cause problems such as malnutrition, poor growth, loss of muscle mass and decreased immunity.  

The recommended daily allowance (RDA) for protein is 0.8g per kilogram of body weight. However experts say older adults, pregnant women and children require more. For seniors, this includes 20 to 30 grams of protein per meal, along with 12 to 15 grams per snack, for optimal muscle health. For the best advice, check with your healthcare provider to ensure you are getting the proper nutrients you need.

Chronic Inflammation Contributes to Many Diseases

The Harvard Medical School writes that many people suffering from fatigue, joint pain, and digestive issues may have chronic inflammation that is affecting their health and not know it. For many, chronic inflammation plays a central role in some of the most challenging diseases of our time. These include conditions like rheumatoid arthritis, cancer, heart disease, diabetes, asthma and even Alzheimer’s disease. 

So, what is inflammation and when is it considered chronic? The Cleveland Clinic explains, “Inflammation is a normal part of your body’s response to injuries and invaders (like germs). It promotes healing and helps you feel better. But inflammation that happens when there’s no injury or invader can harm healthy parts of your body and cause a range of chronic diseases.”

Chronic inflammation can be the result of a variety of factors such as persistent infections, autoimmune disorders, environmental exposures, and lifestyle factors like poor diet and lack of physical activity. In some cases, it may be the result of unresolved acute inflammation, or a direct consequence of conditions like obesity or certain diseases. It may also start as the body’s natural response to an infection or injury but doesn’t shut off as it should, but rather provokes the inflammatory response repeatedly. 

Chronic inflammation generally is not as obvious as acute inflammation. Along with fatigue and low energy, symptoms of chronic inflammation may include depression; constipation, diarrhea, and other gastrointestinal complaints; changes in weight or appetite, headaches and brain fog. To help with symptoms, it is important to get a good night’s sleep, manage stress, maintain a healthy weight and diet, and stay physically active. 

When medical intervention is necessary, experts say medications to treat the symptoms of chronic inflammation include aspirin, a nonsteroidal anti-inflammatory drug; as well as newer, more advanced targeted treatments. Talk to your healthcare provider if you have concerns.

Pneumonia in Older Adults

Every year we hear about the thousands of people who die from pneumonia. The Centers for Disease Control & Prevention (CDC) reports that in 2022 there were over 40,000 deaths in the US due to pneumonia and most of them were over the age of 65. Weakened immune systems, a greater prevalence of chronic diseases, and age-related changes in lung function all contribute to making the elderly more vulnerable to infections including pneumonia. 

The CDC ranks pneumonia as the 10th leading cause of death with a mortality rate for severe pneumonia in the elderly as high as 20 percent. The Cleveland Clinic explains pneumonia is inflammation and fluid in the lungs caused by a bacterial, viral or fungal infection. Symptoms include a fever, difficulty breathing and a cough with yellow, green or bloody mucus. The flu, COVID-19 and pneumococcal disease are common causes of pneumonia; and treatment is based on the cause and severity. 

Bacterial infections include Streptococcus pneumoniae, and are one of the most common forms of pneumonia affecting the elderly. Viruses like the common cold as well as COVID-19 can be the cause of viral pneumonia as well as a secondary bacterial lung infection. Experts explain that while bacterial and viral pneumonia is contagious, many elderly people give themselves pneumonia. They say this is because Streptococcus pneumoniae and other variations of bacterial pneumoniae are already in our respiratory tract. 

William Schaffner, M.D., medical director of the National Foundation for Infectious Diseases and professor of preventive medicine and infectious diseases at Vanderbilt University School of Medicine, explains, “Frail elders often can’t clear secretions from their lungs, and those secretions tend to go down into the bronchial tubes. The area fills with pus, mucous and other liquids, preventing the lungs from functioning properly.” This causes oxygen to be unable to reach the blood and cells. Complications include sepsis in addition to fluid and infection. 

Antibiotics are used to treat bacterial pneumonia. If viral, doctors use anti-viral medications, and if called for, may administer fluids or oxygen therapy. To help prevent a loved one from getting pneumonia, Dr. Schaffner recommends getting an annual flu shot, a pneumococcal vaccine and keeping up with your COVID-19 booster shots. He says it is also important to maintain a healthy lifestyle including a healthy weight; and if you smoke, quit, and maintain good oral health.

As We Age, Regular Health Screenings Are More Important Than Ever

As we age doctors recommend seniors get some additional health screenings. However, certain tests may no longer be required for very elderly people so be sure to talk to your doctor about what is appropriate based on your medical history and age. 

The American Academy of Ophthalmology recommends adults 65 years and older get a thorough eye exam every 1-2 years as problems increase with age. Ophthalmologists warn serious diseases such as glaucoma can silently steal our vision without noticeable symptoms. An exam also looks for signs of cataracts, age-related macular degeneration and diabetic retinopathy. 

Colorectal cancer screening is recommended for adults starting at age 50 and continuing until age 75 (if you’re older than 75, it may depend on your history). Tests include colonoscopies, fecal immunochemical tests (FIT), or flexible sigmoidoscopy.

Breast cancer screenings are recommended at age 50 to 74 every 1-2 years, or based on family history of breast cancer. For men, prostate cancer screenings recommendations vary. The American Cancer Society says, “The decision should be made after getting information about the possible benefits, risks, and uncertainties of prostate cancer screening.” 

Lung Cancer screening is recommended for people at high risk or a history of smoking. Skin cancer checks by a dermatologist are recommended for seniors at least once a year. A bone density test to screen for osteoporosis is recommended for women age 65 years and older, and for men aged 70 and older. Repeat scans should generally be performed every 2 years.

As the prevalence of diabetes increases with age, diabetes screening is important. The Cleveland Clinic recommends screening for diabetes at age 45 and over, with follow-up tests every three years. Those with a BMI greater than 25 and additional risk factors may need more frequent testing.

Hearing screenings are recommended every 5 years until age 65, every 1-3 years after 65, and more frequent testing if there are risk factors or problems. Test results are used to monitor hearing changes over time and provide a picture of a person’s hearing health.  

Other routine screenings include cholesterol and blood pressure as well as annual cognitive and mental health assessments. Recommended immunizations for older adults include influenza vaccines annually, pneumococcal vaccines at age 65 (with a possible booster) and a two-dose shingles vaccine over 50.

We have all had the experience of sitting in the waiting room of a doctor’s office with a clipboard or tablet loaded with forms ready to be filled out. You may wonder why as you have already filled out similar forms at your other healthcare providers, and maybe even at this office.

Experts say it is good to remember there are several reasons why you are being asked for your medical history again. To start with, your health may have changed. Additionally, not all doctor offices have access to the same digital information; or it may not have been updated. Also, a specialist may be looking for different information not typically collected by your primary care physician. Remember your refusal to answer questions could make it harder for your doctor to see the full picture of your problem; or at the very least make you look uncooperative. Dr. Robert Shmerling, a rheumatologist and senior faculty editor at Harvard Health Publishing, agrees you are not obligated to fill out your medical history but says the response from the practice might be, “How can we provide the best care if you don’t provide the information?”

So what is the most important information to include? The Harvard Health Publishing article highlights the most useful things to share with your healthcare provider:

  • chronic or new symptoms and conditions

  • past surgeries

  • family medical history

  • insurance information

  • current prescription and over-the counter medicines, supplements, vitamins, and any herbal remedies or complementary medicines you use

  • medication allergies

  • vaccination history

  • any screening tests you’ve had, so they won’t be prescribed unnecessarily

  • any metal implants you have, which could affect screenings.

Lesser-Known Things That Are Linked to Cognitive Decline

As we age, more of us worry about ourselves or a loved one getting Alzheimer’s disease. Currently the disease does not have a cure but there are several medications that help mitigate symptoms, and a few more promising treatments on the horizon. That being said, it is still a disease none of us wish to experience.

Many people are aware of studies showing a good diet and exercise are not only good for heart health but also good for maintaining cognitive health. Additionally, Jessica Caldwell, a neuropsychologist and director of the Women’s Alzheimer’s Movement Prevention Center at the Cleveland Clinic, told AARP there are lesser-known things people can do to strengthen cognitive health. She explains it involves letting go of some bad habits and replacing them with healthier ones. 

One suggestion is to try and limit negative thoughts. A study reports “participants who repeatedly dwelled on negative thoughts had more amyloid and tau deposits in their brain, the biological markers of Alzheimer’s disease.” While we all have spent time thinking about bad things happening or that have happened, experts say repeatedly focusing on negative thoughts may put us at greater risk for Alzheimer’s.  

Another easy way to lessen your risk for Alzheimer’s is to get vaccinated. Paul Schulz, M.D., a neurologist at McGovern Medical School at the University of Texas Health Science Center in Houston, reported on a nationwide study, which found adults over 65 who received at least one influenza vaccination were 40 percent less likely to develop Alzheimer’s disease. Another study found a 25 to 30 percent reduction in Alzheimer’s for seniors who had a pneumonia vaccination. 

Other studies link an increase in the risk for Alzheimer’s to consuming sugary drinks (including fruit juices – better to eat the fruit). Researchers say it can lead to spiking blood sugar and an exaggerated insulin response, which may trigger chronic inflammation in the brain.

How Much Water Do We Actually Need Each Day?

We know we need water to survive but in terms of just how much, there are a lot of opinions regarding the perfect amount we should be consuming each day. Experts say the answer is not straight forward, and a recent article in Popular Science weighs in with yet one more opinion to, as they put it, a surprisingly complex issue.

While we all have gotten used to reading that we should be jugging down eight cups of water each day for optimal health, recently researchers say that is not really the answer. Rather, it appears commonsense is the truest answer as experts say there are a number of factors to take into consideration regarding how much water our bodies need each day. Variables include how much you exercise, how hot or cold is it outside, what food you consume that day, and what type of health issues you have or medications you take. This is why 8 cups of water may or may not be the amount your body needs. 

The best answer appears to be to listen to your body’s thirst sensation for when you need more hydration. The Harvard Medical School says on average most people need about four to six cups of plain water each day along with their food intake. 

However, it is important to note that for older adults the thirst mechanism is blunted and may not be as effective at letting them know when they are dehydrated. This can be due to reduced kidney function that increases fluid loss. Other factors are medications such as diuretics or reduced mobility making it harder for  seniors to access fluids. People with dementia may have a difficult time recognizing thirst or no longer have the ability to express their need for hydration. If you are at risk for dehydration, talk to your healthcare provider about strategies to ensure you get enough fluids throughout the day.
