How Much Water Do We Actually Need Each Day?

We know we need water to survive but in terms of just how much, there are a lot of opinions regarding the perfect amount we should be consuming each day. Experts say the answer is not straight forward, and a recent article in Popular Science weighs in with yet one more opinion to, as they put it, a surprisingly complex issue.

While we all have gotten used to reading that we should be jugging down eight cups of water each day for optimal health, recently researchers say that is not really the answer. Rather, it appears commonsense is the truest answer as experts say there are a number of factors to take into consideration regarding how much water our bodies need each day. Variables include how much you exercise, how hot or cold is it outside, what food you consume that day, and what type of health issues you have or medications you take. This is why 8 cups of water may or may not be the amount your body needs. 

The best answer appears to be to listen to your body’s thirst sensation for when you need more hydration. The Harvard Medical School says on average most people need about four to six cups of plain water each day along with their food intake. 

However, it is important to note that for older adults the thirst mechanism is blunted and may not be as effective at letting them know when they are dehydrated. This can be due to reduced kidney function that increases fluid loss. Other factors are medications such as diuretics or reduced mobility making it harder for  seniors to access fluids. People with dementia may have a difficult time recognizing thirst or no longer have the ability to express their need for hydration. If you are at risk for dehydration, talk to your healthcare provider about strategies to ensure you get enough fluids throughout the day.

The Centers for Disease Control and Prevention (CDC) recommends at least 150 minutes of moderate-intensity physical activity each week and at least two days of strength training. While you may think you don’t have time to go to a gym to workout, experts say many strength training exercises can be done at home using your own body weight, resistance bands and light weights. For adults dealing with obesity, better weight loss results can be achieved with a combination of strength training, aerobic exercises and dietary changes.

Experts say as we age, our bodies may start to feel less stable due to things like a loss of muscle mass, lack of flexibility and slower reflexes. Another culprit interfering with balance is inner ear problems. The labyrinth is a part of the inner ear that controls balance and when it becomes inflamed or infected, it can cause problems including vertigo. The condition called Labyrinthitis generally is a result of other issues such as head injuries, viral or bacterial infections, and circulatory system conditions like strokes or low blood pressure. 

The National Institute on Aging says families and caregivers should be on the lookout for an older person complaining of unsteadiness; the feeling the room is spinning or moving; confusion; blurry vision; and falling. To determine the cause of the symptoms of lightheadedness or dizziness as well as the best treatments available, consult with your doctor, and if needed, seek the care of a specialist.

By Age 75, Almost Half of Seniors Have Difficulty Hearing

The Harvard Health Publishing reports that approximately one in three people age 65 to 74 have some level of hearing loss. By age 75, almost half of seniors have difficulty hearing. Experts say while common, ignoring a decline in hearing can have negative consequences.

One issue is evidence linking age-related hearing loss to a higher risk of cognitive decline. Researchers say hearing loss may impair new nerve cell creation in the hippocampus, the brain’s memory region. Moreover, hearing loss is believed to increase a person’s risk for depression and social isolation; and is also linked to a person being less active.

The Harvard article says if you think you need a hearing test you probably do. Dr. James Naples, assistant professor of otolaryngology–head and neck surgery at Harvard-affiliated Beth Israel Deaconess Medical Center, explains it is hard to “differentiate the cause of one’s hearing loss and its severity without an evaluation.”

Age-related hearing loss, also known as presbycusis, can be caused by several things with the most common being a change in the inner ear that alters the nerve pathways from the ear to the brain. The Mayo Clinic suggests getting a base-line hearing test by the age of 65. Talk to your primary care provider as they may have the equipment to schedule a test or can refer you to a certified audiologist to evaluate your hearing. 

The CDC explains a mild hearing loss typically allows most speech sounds other than very soft ones to come through. A moderate hearing loss may mean the person is unable to hear most speech at normal levels. Doctors say age-related hearing loss and noise-induced hearing loss generally affect both ears. The Alzheimer’s Association explains, “sundowner’s syndrome” or “sundowning” is a set of symptoms or behaviors that are found in some people with Alzheimer’s disease and dementia. The syndrome’s symptoms include agitation and anxiety, and it occurs in the later part of the day and evening with marked behavior changes in the person.

Fred Kobylarz, MD, co-director of the Center for Healthy Aging at Rutgers Robert Wood Johnson Medical School says it is not a disease, but rather a symptom of dementia. While doctors do not know the cause of sundowning, they believe it may be a “combination of hormonal changes, brain deterioration or damage that has occurred, environmental factors, disruption to a person’s circadian rhythm, and more.”

The syndrome may start as early as 1 p.m. in some people but usually is later in the day. Along with agitation and anxiety; symptoms can include things like pacing, disorientation, hallucinations and problems sleeping. Other behavior changes may include angry outbursts and aggressive, sometimes even physical altercations. Others just want to be closer to a person they trust as needed reassurance. Some people do not remember what happened earlier in the day and even forget they just ate dinner and want to eat again. 

Experts say in some cases sundowning starts before a person has had a diagnosis of Alzheimer’s or another form of dementia. However, it is not only found in people with dementia as some older adults exhibit symptoms during an illness or period of delirium.    

Doctors recommend families who see symptoms of sundowning to consult with their healthcare provider to rule out other causes. Guidelines for managing the syndrome include setting up a schedule that adheres to their needs and maintaining a consistent daily routine to reduce confusion and fatigue. During morning hours encourage the person to do activities that increase their exposure to natural light to help support better sleep-wake cycles. Avoid over stimulating the person in the evenings and don’t subject them to crowds, bright screens or loud noises.

Modifications in Lifestyle May Lower the Risk of Type 2 Diabetes

Type 1 and Type 2 diabetes affects millions of Americans, and every year more than a million people are diagnosed with the disease. The American Diabetes Association (ADA) reports in 2021, 38.4 million Americans, or 11.6% of the population, had diabetes. Of that number 2 million had type 1 diabetes, which includes more than 300,000 children and adolescents. 

Seniors aged 65 years and older make up a large percentage of Americans with the disease at 29.2% or 16.5 million seniors (diagnosed and undiagnosed). Diabetes can be deadly and was the 8th leading cause of death in 2021. People with diabetes are at an increased risk of vision problems, heart attacks, nerve damage and stroke. In 2022, the CDC estimated the cost of diabetes was $412.9 billion. 

Prediabetes is another huge problem and numbers from 2021 report 97.6 million Americans aged 18 and older are affected. It is a chronic condition in which blood glucose levels are higher than normal but not high enough to be classified as diabetes. While it carries an increased risk of developing type 2 diabetes, it isn’t inevitable. Studies show prediabetes can be reversed or the risk mitigated with lifestyle changes such as diet, medications, and early detection. 

Yale Medicine endocrinologist, Anika Anam, MD, explains, “Prediabetes goes hand in hand with metabolic syndrome, the term for a condition that includes hypertension, obesity, and high cholesterol.” She adds each of those conditions raises the risk for serious issues including heart disease, stroke, and cancer. 

The CDC’s national Diabetes Prevention Program (DPP), finds “people with prediabetes who lost a modest amount of weight—5 to 7% (10 to 14 pounds for a person weighing 200 pounds, for example)—with the help of a structured lifestyle-change program cut their risk of developing type 2 diabetes by 58%.” In addition, experts say at least 30 minutes of moderate exercise per day can not only improve insulin sensitivity and lower blood sugar levels, but aid in weight loss. Cutting out added sugars, eating more complex carbs (vegetables, legumes, nuts, and whole grains), and increasing protein are other recommended lifestyle changes as is reducing stress.

In addition, there is a chance of a plastic water bottle sitting in direct sunlight getting hot enough to spark a fire.  High heat can also affect the flavor of carbonated soda, and there have been cases of cans or bottles exploding due to high temperatures creating pressure inside the container.  

Batteries and devices such as phones, cameras and laptops with batteries (and other components) do not do well in hot vehicles either as they can leak and fail. Credit cards are also susceptible to high heat as they may warp or become deformed and unusable. Aerosol cans containing hairspray, paint or other products are labeled safe for storage under 120 degrees Fahrenheit and could explode in higher temperatures, which is possible in a parked car sitting in the sun on hot days.

Other items that can be affected by high heat include prescription and non-prescription medications. Experts say medications should be stored under 77 degrees (unless it is a medication that needs to be refrigerated) as heat can cause it to lose its effectiveness. If you do need to have items in a hot car, it is helpful to have an insulated bag or chest to keep something cool for a short time. Moreover, most importantly remember it is not safe to leave children, pets or the elderly in a car parked in the sun for any length of time.
